Antiochian Events strongly recommends all registrations be processed online, especially with credit card payments. If you have difficulty in processing transactions, please submit a Support Ticket.

Below is more information on the essential paper forms for the 2025 PLC:

Registration Forms: pre-registration will be available online only. There will be a paper Event Registration form available at the PLC, however, several events do sell out before the PLC begins so it is recommended that you register online. The early registration deadline is May 25th. After this date, event prices will increase. Click HERE to see a comparison.

  • Souvenir Journal: this form includes pricing for a specific category and space to include text or artwork. If you wish to email electronic files or artwork, use the email address on this form. Click HERE for page layout and measurement in both English and Metric Systems.

  • Vendor Form: Registration will only be available online this year for those interested in participating as a vendor or having a display booth. All Vendors must be approved by the Archdiocese, through the Department of Convention and Conference Planning, to participate as a Vendor at any PLC’s or Convention.

Supplemental Forms: the following forms are not available to be filled out online. They are required for all minors attending on-site and off-site events, regardless if parents, guardians or chaperones are in attendance at PLCs.

  • Minor Participation: required for all minors. The form must be signed by parents, guardians or chaperones, plus all minor children under the age of 18, indicating that they all understand the rules of PLCs and Conventions. The form must be submitted for all minors to receive their registration badges.

  • Kid’s Club Medical: Required to be filled out by parents, guardians or chaperones for all children ages 4-12 wh the Kid’s Club.

  • Young Adult Off-Site Events (Thursday/Saturday): These forms are required for all attending either offsite event, regardless of age. No one can attend these events without first submitting the respective signed form (one per Person).
